Shift Change Bedside Safety Checklist

Hi PICU staff:  This morning I introduced our new bedside safety checklist to be completed at shift changes.  My sincere apologies for interrupting report. 
There have been several safety concerns brought to our attention, many of which should have been caught during 2 RN checks and shift change.  The purpose of the checklist is to get everyone back in the habit of performing bedside safety checks before assuming care of each patient.  We're all human, we all make mistakes, we just don't want those mistakes to ever reach our patients. 
The checklist includes:
Infusions: correct dose/rate- calculated by hand
IV site assessed; connections secure; tubing/bags not expired
Bag and mask present and functioning
Suction present and functioning
ID band ON patient
Alarms on, limits written down
I will be arriving early all week to help initiate the safety checks.  My hope is after a few weeks, once we are in the habit of double checking these things, we can do away with the paper forms and simply sign/cosign in McKesson "safety check complete".
