Request from Jill Thompson

Attn Unit Secretaries, PCTs and RNs:
There has been a change in the Medication Reconciliation form the MDs fill out for new admissions.  This form is now a part of the History and Physical, and therefore, must be filed there.  When chart audits are done, if the form is not there, we get dinged.  It is most commonly being filed incorrectly under Orders.  Before filing this form, it should be faxed to pharmacy, then signed/dated.  So how can you help?
Unit Secretaries/PCTs: If you come across this form while putting the chart together, please fax it to pharmacy, sign and date that you have done so, and file it under History and Physical.
RNs: If you see this form in your Orders, please fax it to pharmacy, sign and date that you have done so, and file it correctly under History and Physical
