New Roles in Case Management and Social Work

FYI: The roles of the Nurse Case Manager and Utilization Review Nurse have merged into one position. There have been inservices to best prepare each member for the additional role responsibilities. During this first month of transition, a nurse case manager will be paired with a utilization review nurse to serve as a resource for each other. Please bear with us as we are taking on this new endeavor.


The Social Workers will be participating in the discharge planning process as well as reducing readmissions and decreasing length of stay while performing their traditional role. Below is a list of potential SW interventions.


The Social workers will be partnered with the Nurse Case Manager to best achieve the above goals for our patients


Jeralyn Wunderly (2-4288/ pg 12259)is the Nurse Case Manager for PICU/7C and Jennifer Reyes (2-9460/ pg 13263) is the Social Worker for PICU/7C. I will post a phone list on each unit for the other service lines in CH.

Social Work interventions include:

Funding issues

Custody Issues/Family Dynamic Issues/Legal Issues

Non-compliance

Abuse/Neglect & Other DSS Issues

Resources: WIC, SSI,Child support, basic newborn needs, etc

Drugs/Alcohol (Counseling/Resources)

Complex Psychosocial Issues

Adoptions

Late/Scant Prenatal Care/Teenage Pregnancy

Traumas

New Chronic Diagnosis

Lodging/Meals/transportation


RNs, Case Managers and Social Workers will communicate to determine who does what for each patient and work as a team to accomplish goals.
