Central Lines

We have had several issues with Central Lines this week. I'm planning some inservices for when I return from vacation, but in the meantime, here are a few reminders/clarifications: If the sutures or stat-lock of a central line are not secure, the attending must be made aware. On nights, the resident on-call must be notified to re-suture or to notify the attending if they are not comfortable re-suturing. When placing a biopatch on a central line, please DO NOT circle the lumen with it, but simply place it on top of the insertion site. Circling the lumen may make it look better or keep it in place when placing the tegaderm, but it also makes the next dressing change more difficult. Just last night a PICC line came out because the biopatch was caught on it. Dr. Evans will be performing a lit search to support her preference of using stat-locks instead of sutures. For now, we will be seeing both sutures and stat-locks per physician preference. I will consult the VAIN team and do some policy searching to determine clear standards for care of lines with stat-locks, but for now, please notify an MD if the stat-lock is not secure. When in doubt- have the attending look at your line. They would much rather secure it than have to place a new one. Thanks everyone, see you August 26th!

  1. I wish that unitl there is concrete evidence that the Statlock reduces infection, we were able to continue to suture our lines in. From a nursing and personal standpoint, I do not like Statlocks, and feel that when I am changing the dressing, the line always seems to pull out to some degree and requires a second set of hands to change... more exposure...

  2. We do have concrete evidence, the CDC actually recommends sutureless devices. A study was done in our own PCICU as well, and they now use statlocks. I have a feeling we will be seeing more of them. I am planning inservices to practice changing them next week, hoping this makes everyone more comfortable. Please continue to voice your concerns, and I will relay them to the team.
