Pneumoccal Immunization Screening

Please remember to screen all patients 5 years and older for the pneumoccal immunizations.  Only the high risk populations need to be asked about vaccination, but you need to screen everyone over 5.  PICU please remember to do this on any patients being discharged.  Diabetic patients are in the high risk category and we often discharge them within 24 hours, so please do not forget this important step.  It is part of the admission process, so please remember to hit show all and to go back each shift to check that the admission screening has been completed.
