My Story/Our Story

As mentioned previously in staff meetings, we are starting a new process to develop more personal relationships with our patients and families.  My Story/Our Story consist of a few questions about the patient, the family, likes, dislikes, etc.  RNs will ask the questions and fill in the answers on the My/Our Story form upon admission and display it at the patient's bedside.  The goal would be for all health care providers to look at that form before interacting with the patient or family to get to know them a bit better.  This process greatly increased patient satisfaction in the pilot unit.  In PICU, Ms. Carrie placed an Our Story form in each blank kardex to remind the RN to fill it out.  On 7C, the forms are with all other paper forms.  Let me know if you have any questions.
