Noncompliance with correct phone numbers for discharge callbacks

Please, please, please when discharging a patient, ask the parents for the best contact number and write that number on the discharge paperwork.  We are less than 70% compliant with this!!!  We are trying to improve our patient satisfaction and offer a follow up phone call to the families to give them the opportunity to ask questions.  We now have a centralized call center that does the discharge phone calls for us.  It is imperitive that we put the correct number so the families have been contacted.  Also please make a note if the contact is a FOSTER Parent.  They have had situations where the biolgical family was called post discharge instead of the foster parent  (you can image the joy of that conversation!).
