Success Factors Annual Evaluations for Staff
Hey guys,
From the flood of Success Factors emails in your inbox, I'm sure you have all realized it is time for annual evaluations. Please take a moment to complete your 360 Peer evaluations. These are completely anonymous! If you do not wish to participate, please still login and select "Decline to Participate". The peer eval will not show as completed until everyone has either answered or selected the decline option. This causes a time delay in me being able to collate the information, review the employee self eval, and then add my own comments for the final rating.
All 64 of you must be done by August 31st. That means I need to complete the evals ASAP to have enough time to meet with each of you to review. I really want to have at least 20-30 minutes with you all to review the evaluation, set goals for next year, discuss professional development goals (we all have to have them for next year....this is new), and to learn from you how I can better meet your needs and those of the unit. So, I am asking that you complete all 360 evals by June 30th. That will give me two months to get everything done. I will also be moving into my new house at some point during July/August and taking some time off. I will not ask any of you to come in on days off to review your evaluation, but will coordinate a time when you are here. This includes night shift, so I will be staying late and coming in early for most of the next two months.
If you did not complete your self review by the May 31st deadline and wish to do so, please email me and I can send it back to you for review with a new deadline of June 30th. After that, I will just have to go with peer evals and my experiences with you over the past year. Please remember that MENTOR means you excel in that area and are able to mentor others and that MEETS/SOLID means you are great too, just maybe not at a level yet to be a resource to others in that area. DOES NOT MEET is self explanatory.
Remember to get all of your CATTS, TB, etc. done by June 30th or you will have to be marked DOES NOT MEET in job competencies. This means you would automatically NOT be eligible for any merit raise that may be offered. Being aware of our current financial strains, we are all assuming there will be no merit raises offered, but that has not yet been announced. Unit goals count for 25% of your score. For 7C those goals were patient satisfaction and hand hygiene. For PICU, those goals were hand hygiene and CAUTI. We are on the cusp of meeting the 90% hand hygiene goal, so WASH YOUR HANDS!!! That is a gimmie goal that we should meet everyday even when no one is looking! We should score well with patient satisfaction and CAUTI.
Please let me know if you have questions or concerns. I can tell you as a team, you guys are ROCKSTARS!!!!!!!!!!!!!!!!!
Thanks so much for making me the luckiest Nurse Manager!

Lots of Updates!
PEWS: We are rolling out Pediatric Early Warning Signs (PEWS) on 7C beginning Monday June 24. I've gone over the details with a few of you and others may have used these when floating to 7E or 7B. This is a paper form for now, but after the trial the safety team will try to have it put in McKesson. It will DEFINITELY be in EPIC. I have placed laminated copies of the algorithm in each wallaroo. It walks you through exactly what to do depending on your patient's score. Scoring is done at least Q4, more often if their score is higher. Please page Tanya Green with any questions at 13408. She will be around Monday to help roll these out.
Labeling: In safety rounds over the last few months we have been discussing the need for labeling of all lines, tubes, and drains to prevent accidental misuse. The issue is being taken to practice council in hopes of creating a hospital-wide policy. In the meantime, it was decided to implement this practice in PICU and 7C as we attempted to do months ago. Please label all tubes and drains with grey OTHER labels or white tape. This includes foleys, NG, GT, Chest Tubes, JPs, Peritoneal drains, etc. Let me know if you have any questions.
Allergy Acknowledgements: Allergy acknowledgements are now being audited and PICU and 7C are missing quite a few. Please get in the habit of clicking on the RED allergy icon (Stop sign w/ !) and making sure each allergy says CONFIRMED. I posted screen shots on both education boards for clarification. If you have questions about this, please let me know.
Resident Skills: Did you know that Residents need to learn how to place IVs, foleys, and other nursing skills? Let them do your work for you! Dr. Evans created a survey for third years to determine their comfort level with performing necessary skills and two of the skills residents are least comfortable with are PIV placement and foley placement. Help them help you by offering to show them how to do it, or allow them to perform the skill with your guidance. Please don't hand them the foley kit and walk away, but take advantage of their help and engage them so they learn. If you meet resistance with this, please let myself or Dr. Evans know.
Thanks Everyone!

CATTS and TB due by June 30th - Please complete ASAP
If you are on the list below, please complete your CATTS as soon as possible. After June 30th, you will be marked for not completing unit competencies. This is a very small list and I truly appreciate everyone getting busy on this before the last week of June!!! Great job!
There is now TB serum on both units. Please get your PPD placed as soon as possible. Even if you are not due in June, just go ahead and have it done so you can be on the same schedule as the rest of the unit next year. It will not hurt you to have it done again. Thanks to Amelia for getting the serum on the units. What a win! Remember the PPD has to be read by June 30th, so you much have it placed by June 27 or 28.
Thanks so much!

SBS and WAT
PICU- you may have noticed the State Behavioral Scale (SBS) is now under vital signs in McKesson, yay! Just to clarify: this scale is to be used on every MECHANICALLY VENTILATED patient receiving IV SEDATION. Here is the link to the creator's website for more info: http://www.marthaaqcurley.com/sbs.html
This scale is to be used at least Q4 (yes, even on chemically paralyzed patients), and before and after bolus sedation is given. I have placed laminated copies of the scale in each bedside chart for reference.
The WAT-1 scales are also now in McKesson under the 24 hour assessment. You can find it between Neuro and Special Neuro if you select "show all" to the far left where the systems are listed. When planning for extubation, note how long your patient has been on drips- more than a few days? Ask the team if they would like WAT scores. >5 days?? DEFINITELY complete WAT scores. WAT scores should be completed Q4 starting with the FIRST wean (usually prior to extubation) and continuing until 72 hours after their LAST dose (usually enteral) of sedation.
Any questions, please let me know, the builders can always fix/tweak things that aren't working. Thanks!

TB Test
All Staff,
I beg, borrowed, and stole 3 TB solution vials from employee health for our units. There are ONLY three vials which will produce approximately 50 total skin tests, and once the vials are gone you will have to go to Employee Health. The supplies will be in the unit refridgerators. Please remember that everyone must complete a skin test and have the documentationt to Amanda by the end of June! Thanks,

Attendance
In my recent review of attendance since January 1st, many of our staff have exceeded the limits per hospital policy. I will be following up with those offenders this week with the necessary disciplinary action. Some of you who are at 3 occurrences have received an email warning. Please remember the policy is no more than 3 attendance ooccurrences in a rolling 6 month period. FMLA is excused. Also, it is NOT true that an MD excuse will cover you. Unless the absence is FMLA related, it still counts as an occurrence. Please let me know if you have questions.
Thanks, Melinda

Our Hospital Goal is 80% participation. Our unit goal is 100%. We are currently between 55 and 60% for PICU and 7C. Please help me get the most information so we can make real plans to improve our units in the coming year. I truly want 7C & PICU to be the BEST places to work in the CH. You guys already give the best care...as demonstrated again this week with wonderful teamwork in both areas! Ya'll make me proud each day.
Thanks,
Melinda
MEDICATION SAFETY ALERT!
This was presented in EROC last week. When confirming new med orders, please look at both "versions". The MD enters the order, then Pharmacy re-enters it and the orders show up side-by-side. They should MATCH. If they don't, please call Pharmacy to correct. See link below for a screen shot (it makes sense when you see it). A physician ordered Valproic Acid, and the Pharmacy ordered Vancomycin... yikes!
